Winter teams must complete a Winter Team Registration Form similar to the one required for Spring teams. CLICK HERE to retrieve the form. Coaches: Every person wishing to serve as a coach or program administrator MUST complete an Employment/Volunteer Disclosure Statement. "Coaches" include head coaches, assistant coaches, and substitute coaches; Program Administrators include Club officers, program directors, team managers, referees, assistant referees, and athletic trainers. The form can be found at the New York State West Youth Soccer Association Web site CLICK HERE. Note: A parent who will be Acting Coach for only one game must complete this process, so if you would volunteer to be a "stand in" when necessary, get your paperwork done! The completed form, along with a check for $6 payable to NYSWYSA is mailed to the Dryden Soccer Club Registrar, P.O. Box 277, Dryden, NY 13053. Thank you! All coaches are required to REPORT any yellow and/or red cards incurred by any player in order to assure compliance with the Code of Conduct the Club now has in place. All players, parents and coaches must read the Codes of Conduct, sign and submit these forms. The Coach hangs onto the players and parents copies, the coach's copy goes to the Club Vice President, Tim Wiese. The Youth Soccer Age Matrix has been updated for the 2007-2008 year to help determine for which age groups a youth player is eligible. Want to know about registering a team? The registrar has prepared a description of the registration process. The 2004-2005 Registration Forms are available now - follow the "Registration Forms" link at left. There are now two separate forms for travel and rec league registration. We have a Deposit Form for use by all teams. This will help our Treasurer provide the reports that are being requested by coaches and managers. It is available here in PDF format.
